  2. Enter the LLC File Number
  3. Enter the NEW Registered Agent's name and/or address
  4. Enter Manager's name and capacity
  5. Review, submit, and pay

Submit it by mail

  1. Print and sign the form in DUPLICATE
  2. Payment may be made by check payable to Secretary of State
  3. Mail the documents to: Department of Business Services Limited Liability Division 501 S. Second St., Rm. 351 Springfield, IL 62756
