8. Name and address of all applicant(s) or manager(s)
9. Name and address of the parent company (optional)
10. Do not enter information for an agent(s)
11. Veteran Exemption - Check the box if a Veteran
12. Select a service option
13. Review filing
14. Signature page - Check the boxes. Add your full name and click "Today" as your signature.
15. File online and pay.
