7. Use the first manager as the authorized agent. Save and Continue.
8. Review the document pdf and PRINT NAME in Print Name box underneath the signature.
9. Verify for Checkout, click Select Filings and, checkout.

1. Print and complete the form
2. Include a $25 check payable to "Secretary of State"
