3. Entity type
4. The state of jurisdiction
5. The LLC SCC ID number
6. Date of filing
7. Sign as an Individual
8. Digital/Wet signature and Printed or typed name of the authorized person. Date of filing.
9. Title is Manager
10. Contact Information (Email address and phone number)

Submit it online

To register an assumed name (DBA) online, follow the instructions below:
1. Log in
2. Click Online Services then "NAME RESERVATIONS/REGISTRATIONS/FICTITIOUS NAME"
3. Name type - FICTITIOUS NAME
4. Filing type - FICTITIOUS NAME CERTIFICATE
5. Click Yes
6. Locate your business in the system by
either:
A. Searching for it using the Entity
Name field, OR
B. Entering your Entity ID
7. Click the Select button next to your business’ name. DO NOT click the green entity name and continue
9. Once the Entity Information appears, enter the email and phone number
10. The DBA to be registered
11. Digital/Wet signature of an authorized person
12. Printed or typed name of the authorized person
13. Add
14. Review the filing
15. Add to cart and checkout

Submit it by mail
