2. Select "715 - "CERTIFICATE OF AMENDMENT TO THE ARTICLES ORGANIZATION" and continue
3. The first section is pre-filled with current business information
4. If amending the LLC name, state the new name
5. Business purpose
6. Duration of LLC (Preferably perpetual)
7. If amending resident agent, the new resident agent name and address
8. Additional provisions, if any
9. Effectiveness of Filing: Filing date/future date (back-dating is not allowed)
10. Check a box
11. Printed or typed name of the authorized person; Title of the authorized person
