7) Skip to number 9 and add any Additional matters (If applicable)
8) State the amendments to the certificate
9) If the entity will have an Effective Date and time fill in number 11
10) Enter the special filing instructions (If applicable)
11) Enter Filer's contact information
12) Enter the signature of the authorized signatory
13) Click Accept
14) Review, submit and pay
