4) Enter the signature, printed name and title
5) Enter the contact name and phone number

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1) Go to https://secure.sos.state.or.us/ and click on Register a business
2) Enter the login details or create an account
3) Click on "Update"
4) Enter the Registry Number
5) Tick the entity type
6) Enter the email address
7) Select business type from the drop-down
8) Enter the name of the LLC
9) Check 'Yes" for Members or Managers
10) Enter the new Member(s) or Manager(s) name and address.
11) Do NOT check "Yes" for any other option
12) Add signature, title, and name
13) Enter the contact name and phone number
14) Submit

Submit it by mail

1. Print and fill out the form
2. Mail forms to Corporation Division - 255 Capitol St. NE, Suite 151 Salem, OR 97310-1327
