This form is available here for download. Once downloaded, fill it out like so:

1) Enter the the name of entity
2) Enter the date of Formation
3) Enter the entity ID Number
4) Enter the typed name and address of the individual preparing the form
5) Enter the date of amendment
6) Describe the nature of the amendment
(a) List the new name of the LLC
7) Check the box
8) Add date, signature, typed name and title of an authorized representative
9) Fill out the payment form

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1. Go to https://www.sos.alabama.gov/
2. Click on Business Services (under picture)
3. Scroll down to Business Filing Amendments
4. Click "Continue to application" for Non-Subscriber or Login if you are a subscriber
5. List contact information
6. Entity ID Number and search
7. Click select, verify the information and check box
8. Check box for Legal Name
9. List new name and reservation ID
10. Upload previous articles of organization document
11. Review
12. Check box
13. Signature and title
14. Enter credit card information
15. Submit payment
16. The document is obtained immediately after payment

Submit it by mail

1. Print and fill out the form (including the payment form)
2. Attach a copy of the name Reservation Certificate
3. Mail 2 copies of this completed form along with a self-addressed, stamped envelope to: Secretary of State, Business Services, P.O. Box 5616, Montgomery, Alabama 36103
4. Include a check, money order, or credit card payment
