4) Do not do anything
5) Enter the date, filer's name, signature, and capacity

Submit it online

1) Go to the Secretary of State's website: https://apps.ilsos.gov/llcartamendment/
2) Enter the LLC File Number, then select Yes
3) Enter the NEW entity name followed by LLC
4) Select and list the Manager's name and capacity
5) Review, submit, and pay

Submit it by mail

1) Print and sign the form in DUPLICATE
2) Payment may be made by check payable to Secretary of State
3) Mail the documents to:
