2. The new LLC name
3. Date of filing
4. Digital/Wet signature and Printed or typed name of the authorized person
6. Title is Manager
7. LLC's ID

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

To change your LLC name online, follow the instructions below:
1. Log on to CIS at https://cis.scc.virginia.gov/
2. Click Online Services on the top left
3. Under Business Entities, click EXISTING BUSINESSES
4. Select the business type
5. Click "Articles of Amendment - Name Change"
6. Locate your business in the system by
either:
A. Searching for it using the Entity
Name field, OR
B. Entering your Entity ID
7. Click the Select button next to your business’ name. DO NOT click the green entity name and continue
8. Once the Entity Information appears, click Next
9. Entity's email address and phone number
10. New LLC's name
11. Date of filing
12. Check either the first or second option
13. Attachments if needed
14. Digital/Wet signature of an authorized person
15. Printed or typed name of the authorized person
16. Add
17. Review the filing
18. Add to cart and checkout

Submit it by mail

To file for a name amendment by mail, follow the instruction below:
1. Print and complete the form
2. Include a $25 check payable to "State Corporation Commission"
3. Mail the document and check to
