5.a) Registered Agent and Registered Office shown on FL Dept. of State records
5.b) Enter NEW Registered Agent and Registered Office
6) Enter the signature and name of filer
7) Enter the signature of New Registered Agent

1. Print the 6-page form, which includes the cover letter, and sign it.
2. Submit the check made payable to the Florida Department of State for the total amount of the filing fee.
