2) Enter the name of the LLC
3) Enter ONLY the new registered agent's name in section 4
4) Enter ONLY the new registered agent's address in Section 5
5) Enter the signature, printed name and title
6) Enter the contact name and phone number

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1) Go to https://secure.sos.state.or.us/ and click on Register a business
2) Enter the login details or create an account
3) Click on "Update"
4) Enter the Registry Number
5) Tick the entity type
6) Enter the email address
7) Select business type from the drop-down
8) Enter the name of the LLC
9) Check 'Yes" for Registered Agent or their address
10) Enter the new Registered Agent's name and address
11) Check 'Yes" for "Registered Agent has consented to this appointment." (If applicable)
12) Check 'Yes" if the entity has been notified in writing of this change. (If applicable)
13) Do NOT check "Yes" for any other option
14) Add signature, title, and name
15) Enter the contact name and phone number
16) Submit

Submit it by mail

1. Print and fill out the form
2. Mail forms to Corporation Division - 255 Capitol St. NE, Suite 151 Salem, OR 97310-1327
