1) Go to the Secretary of State's website: https://onestop.wv.gov/b4wvpublic/default.aspx?pagename=filingcabinet and click "Address/Office/Agent Change". Look up the LLC using its name and click continue
2) If amending the Principal Office Address, enter the NEW Principal Office Address on Address Information page
3) If amending the Registered Agent's name and/or address, enter the NEW Registered Agent's name and/or address on "Agent of Process" page
4) If amending the manager's name, you may edit, remove, or add manager's name on Manager Information page
5) Continue to list the contact and manager's information
6) Review the details and pay

Submit it by mail

1. Fill out and print the form including the Customer Order Request
