1. Go to https://www.sos.alabama.gov/
2. Click on Business Services (under picture)
3. Scroll down to Name Reservation Menu
4. Click on Domestic Dissolutions
5. Click "Continue to application" for Non-Subscriber or Login if you are a subscriber
6. List contact information
7. Entity ID Number or name of entity and search
8. Click select, verify the information and check the box
9. Effective date (If applicable)
10. Reason for dissolution
11. Upload attachment (If applicable)
12. Review
13. Check box
14. Signature and title
15. Enter credit card information
16. Submit payment
17. The document is obtained immediately after payment

Submit it by mail

1. Print and fill out the form (including the payment form)
2. Mail 2 copies of this completed form along with a self-addressed, stamped envelope to: Secretary of State, Business Services, P.O. Box 5616, Montgomery, Alabama 36103
3) Include a check, money order, or credit card payment
