Certificate of Cancellation form
1) LLC name
2) LLC Entity (File) Number
3) Check the box if the vote to dissolve was made by the vote of ALL the members.
4) Do not do anything
5) Do not do anything
6) Name and signature
There's multiple ways to submit this form - online, by mail, via fax, in-person.
Submit it online
1) Go to the Secretary of State's website: https://bizfileonline.sos.ca.gov/search/business and look up the LLC using its name.
2) Click on the LLC and then select "File Amendment"
3) Choose "Terminate - CA and Out-of-State LLC"
4) On the Limited Liability Company, Dissolution, and Required Statements tab, select Yes
5) Fill in all the required details.

Submit it by mail
1) Print and sign the form (including the Submission Cover Sheet)
2) Make all checks or money orders payable to the Secretary of State
