7) Select the date of execution
8) Enter the title of the authorized representative and check box
9) Enter the name of the executor
10) Enter the effective date (if applicable)
11) Enter the contact information (optional)
12) Review
13) Pay

Submit it by mail

1) Submit one original to the State of WI-Dept. of Financial Institutions, Box 93348, Milwaukee WI, 53293-0348, together with a check for the $20.00 filing fee payable to the Department of Financial Institutions.
2) For express or priority U.S. mail, please mail to the State of WI-Dept. of Financial Institutions, Division of Corporate and
Consumer Services, 4822 Madison Yards Way, 4th Fl., North Tower, Madison WI, 53705.
