This form is available here for download. Once downloaded, fill it out like so:

1) Enter the LLC name
2) Check box and complete if any one of the following statements apply
3) Enter the effective date if the document is not to be effective upon filing
a) Effective date shall not be later than the 90th day after the filing date
4) Enter the signature date, name of the LLC, signer's capacity, signature and typed or printed name of the signer

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1) Go to https://tnbear.tn.gov/ecommerce/default.aspx
2) Click on "Find and Update a Business Record"
3) Enter the name of the LLC in the search name box. You can also search by the control number
4) Click on the control number
5) In "Available Entity Actions" click on more
6) Click on "Cease Business in Tennessee"
7) Click on "Start Now" in the red box
8) Verify the entity
9) Tick "File Notice of Dissolution (most common)"
10) Check box and complete if any one of the following statements apply
11) Check the box and enter the name and contact information of the authorized representative
12) Pay

Submit it by mail

1. Print and fill out the form
2. The Notice of Dissolution ​should be typed or hand-printed in dark blue or black ink.
3. Make check, cashier’s check or money order payable to the Tennessee Secretary of State.
