(b) Enter the email of business
(c) List documents and return information
1) Enter the name of the LLC
2) Enter the date of Formation
3) Enter the address of DBA
4) Enter the assumed business name
5) Enter the address of the principal office
6) Add signature, date, printed name and title of an authorized representative
7) Add the information of the person preparing the form

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1) Go to http://www.inbiz.in.gov/
2) Click on Online Services then select "all services"
3) Click on "Assumed Name Filing"
4) Search for the entity by name or business ID
5) Click select and continue
6) List the assumed business name, add LLC and check the availability
7) Address of assumed name
8) Effective Date and time: (If applicable)
(a) Effective date must not be later than 90 days
9) Review
10) Check all boxes
11) Signature and Title
12) Add to payment cart
13) Checkout
14) Enter all necessary information
15) Pay

Submit it by mail

1. Print and fill out the form
2. Make check or money order payable to the Secretary of State
3. Mail original paperwork and payment to 302 W. Washington Street, E-018
