13. Title of the authorized person

2. Click on "FORMS"
3. Select "Foreign Registration Statement"
4. Select the business structure of the entity
5. Name of entity in its jurisdiction of formation
6. Name to be used in PA
7. Effectiveness of Filing: Filing date/future date (back-dating is not allowed)
8. Jurisdiction of Formation - Select country and state
9. Select if to get state notifications via email or mail
10. The principal office address (Street and mailing address)
11. Home Jurisdiction Addresses - Select the second box
12. Registered agent address
13. Tax Year or Fiscal Year End - Leave blank
14. Attach any additional information (Optional)
15. Veteran Exemption - Check box if a Veteran
16. Select service option
17. Review filing
18. Signature page - Check the boxes. Add your full name and click "Today" as your signature.
19. File online and pay.
