Fill out LLC 35.22

This form is available here for download. Once downloaded, fill it out like so:

1) List the LLC name as of the date of termination
2) Enter new LLC name if applicable (Form 5.25 must accompany this application)
3) Effective date of termination
4) List the Registered agent's name and address
5) Enter the date, signature, filer's name, and capacity

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1) Go to the Secretary of State's website: https://apps.ilsos.gov/llcreinstatement/
2) Enter the LLC File Number, then select Yes
3) Enter the NEW Principal Office Address (if applicable)
4) Enter Manager's information
5) Select and list the Manager's name and capacity
6) Review, submit, and pay

Submit it by mail

1) Print and sign the form in DUPLICATE
2) Payment may be made by check payable to Secretary of State
3) Mail the documents to:
