14. Name and phone number of the submitter

To reinstate your LLC online, follow the instructions below:
1. Login with entity's CID and PIN; https://cofs.lara.state.mi.us/corpweb/LoginSystem/ExternalLogin.aspx
2. Select "710 - RESTATED ARTICLES OF ORGANIZATION" and continue
3. The first section is pre-filled with current business information
4. Name of LLC (You can change the name if you wish to amend the LLC name)
5. Business purpose
6. Duration of entity (Preferably perpetual)
7. Resident agent name and address
8. Effectiveness of Filing: Filing date/future date (back-dating is not allowed)
9. Additional provisions, if any
10. Check the first option
11. Printed or typed name of the authorized person
12. Title of the authorized person
13. Click accept and add
14. Submitter's information
15. Delivery method for approved documents (Email option is faster)
16. Service option
17. Review and pay

Submit it by mail

To reinstate your LLC by mail, follow the instructions below:
1. Print and complete the form
2. Include a $50 check payable to "State of Michigan"
