6) Choose the third option "Resignation of Agent for Service of Process (§33-44-110)"
7) Enter the agent's name
8) Select who is signing the form
9) Review and pay

Submit it by mail

1. File three copies of this form, the original and either two duplicate original or conformed copies. The Secretary
of State shall mail a copy of this statement to the designated office and also to the principal office.
2. Make a $10 fee payable to the Secretary of State’s Office and mail the document to -Attn: Corporate Filings 1205 Pendleton Street Suite 525 Columbia, SC 29201
