4) Select No under "Has the entity been created in another state or country, unless otherwise stated
5) Entity type is Limited Liability Company
6) Document type is Appliaction to Reserve a Corporate Name
7) Choose an expedite service option
8) Leave Customer Reference Number blank
9) Enter return email address
10) Submit and pay

Submit it by mail

1) Print and sign the form
2) Payment may be made by check payable to Secretary of State
3) Mail the documents to:
