1. Login at https://bsportal.ohiosos.gov/Login.aspx
2. Select "Update an Existing Business record"
3. Search the business name or entity's number
4. Select "Domestic Limited Liability Company Certificate of Amendment or Restatement"
5. Effective date and time is optional
6. Check a box for amendment or reinstatement
7.. New name of LLC
8. Purpose of LLC (This is optional)
9. Add an attachment of any additional amendment (optional)
10. Use the first manager as the authorized agent. Save and Continue.
11. Review the document pdf and PRINT NAME in Print Name box underneath the signature.
12. Verify for Checkout, click Select Filings and, checkout.
