This form is available here for download. Once downloaded, fill it out like so:

1) Entity Information: Put the legal name of the entity.
1.a) SDAT ID number (if known)
2) Enter the Principal Office Address
3) Enter the Registered Agent's name
4) Enter the Registered Agent's address
5) Enter the Member's name and address
6) Do not do anything
7) Check the "LLC has no known creditor(s)" box and enter the date
8) Place the filer's name and signature
9) Enter the Registered Agent's Signature

There's multiple ways to submit this form - online, by mail, via fax, in-person.

Submit it online

1) Go to the Secretary of State's website: https://egov.maryland.gov/BusinessExpress/EntitySearch and look up the LLC using its name.
2) Click the LLC Name and select "Options for this Business"
3) Choose "Cancel/Dissolve Business in Maryland"
4) Select "The company does NOT have known creditors"
5) Enter all necessary details
6) and pay

Submit it by mail

1) Print and sign the form
2) All checks must be made out to State Department of Assessments and Taxation or SDAT
3) Mail documents to:
State Department of Assessments and Taxation, Charter Division
