5) Upload the completed Statement of Dissolution form https://sos.nebraska.gov/sites/sos.nebraska.gov/files/doc/FormSTATEMENT%20OF%20DISSOLUTION_7.01.2021.pdf and pay

Submit it by mail

1. Fill out and print the form
2. Prepare a check and make it payable to Secretary of State.
