6. Digital/Wet signature and Printed or typed name of the authorized person. Date of filing.
7. Title is Manager

Submit it online
To terminate your LLC online, follow the instructions below:
1. Log on to CIS at https://cis.scc.virginia.gov/
2. Click Online Services on the top left
3. Under Business Entities, click EXISTING BUSINESSES
4. Select the business type
5. Click "Articles of Cancellation"
6. Locate your business in the system by
either:
A. Searching for it using the Entity
Name field, OR
B. Entering your Entity ID
7. Click the Select button next to your business’ name. DO NOT click the green entity name and continue
8. Once the Entity Information appears, click Next
9. Entity's email address and phone number
10. Digital/Wet signature of an authorized person
11. Printed or typed name of the authorized person
12. Add
13. Review the filing
14. Add to cart and checkout
Submit it by mail
To file for your LLC termination by mail, follow the instruction below:
1. Print and complete the form
2. Include a $25 check payable to "State Corporation Commission"
3. Mail the document and check to
State Corporation Commission
