3. Select "FICTITIOUS NAME REGISTRATION" and pick an expedited option
4. Enter the DBA name to be registered
5. Name of registrant
6. If the registrant is a business, select yes and enter the entity's number
7. If it's a general partnership, click yes
8. Business purpose of registrant
9. Business address
10. Add attachment, if needed
11. Use the first manager as the authorized agent. Save and Continue.
12. Review the document pdf and PRINT NAME in Print Name box underneath the signature.
