d. Download the form: The current year should be selected automatically. Click "File an Annual Report/Annual Fee/Nonprofit Report". A popup message will appear telling you about how to file by mail. Click the "Continue" button to proceed. This will automatically download your Annual Report to your computer in PDF format.
e. Print the form: Once you have downloaded the Annual Report form, print it out so that you can fill it out by hand.
f. Fill out the form: Provide the requested information in the form, including the name of your LLC, its registered agent's name and address, the names of the members, and the signature of an authorized person.
g. Include the filing fee: As of 2021, the filing fee for an Annual Report in New Hampshire is $100. You can pay the fee by check or money order made payable to the "State of New Hampshire."
h. Mail the form and fee: Once you've completed the Annual Report form and included the filing fee, mail it to the following address:
